EM1804E “Chronic Disease Management and the Shifting Sands of Payment Reform” (IM GR-042018)

The purpose is to introduce the concept of alternative payment models that focus on payment for value-based care, and provide an overview of Southwestern Health Resources, which is an Accountable Care Organization.

Target Audience

UT Southwestern faculty, fellows, residents and medical students, community physicians, nurse clinicians, physician assistants and nurses.

Learning Objectives

At the conclusion of this activity, the participant should be able to:

  • Describe Accountable Care Organizations and Southwestern Health Resources
  • Understand the concept of financial risk as it applies to ACOs
  • Understand the quality metrics and utilization measures as they apply to ACOs
  • Understand how chronic care can be managed in an ACO

Her focus most recently has been establishing the UTSW Endocrinology Clinic at Park Cities Clinical Center and establishing programs for addressing diabetes through her role as a Medical Director for Diabetes in Southwestern Health Resources.
