Purpose and Overview

Sarcoidosis is a multisystem, granulomatous inflammatory disease that can affect the heart. Cardiac sarcoid is a leading cause of death in patients with sarcoidosis and is widely under-recognized by many providers who encounter these patients first in clinical practice. This is likely due to subclinical or asymptomatic cardiac involvement. Historically it was thought that cardiac sarcoidosis affected 5% of patients with systemic sarcoid, however, with advanced cardiac imaging and autopsy data the prevalence is on the order of 25-40%. The purpose of this presentation is to enhance awareness of cardiac sarcoidosis. After this presentation, clinicians will understand when to suspect cardiac involvement in patients with or without known systemic sarcoidosis, how cardiac sarcoid is formally diagnosed, and what updated treatment options are currently recommended including advanced cardiac therapies.

UT Southwestern faculty, fellows, residents and medical students, community physicians, nurse clinicians, physician assistants and nurses.

Learning Objectives

At the conclusion of this activity, the participant should be able to:

  • Recount the most common clinical presentations of cardiac sarcoidosis.
  • Discuss pathways to diagnose cardiac sarcoidosis (definite or probable based on myocardial histology or clinical criteria) and the limitations of each.
  • Understand the four principles of managing cardiac sarcoidosis (treatment of active inflammation, addressing arrhythmias, risk stratifying for sudden cardiac death, management of heart failure)
