Purpose and Overview
Post-acute and long-term care providers have a significant role in the continuum of medical and function supports for older adults and persons with disabilities. Indeed, 42% of Medicare beneficiaries discharged from the hospital receive some form of post acute care. However, most healthcare providers have limited understanding of the scope and funding of these sectors until they or a family member experience a need for services. This presentation will provide an overview of these sectors and the populations served, costs of care, readily accessible information on quality to inform choice, and trends in staffing and financing.
Target Audience
UT Southwestern faculty, fellows, residents and medical students, community physicians, nurse clinicians, physician assistants and nurses.

- Understand the continuum of post-acute and long-term care
- Access and interpret information on provider quality
- Describe trends in post-acute care and long-term care that matter for patients, families and quality
