Abstract: With the development of agenic and generative AI, the introduction of AI-based technologies into health and healthcare is exploding. The capabilities, scope, and number of these new AI-based products is overwhelming many existing ethical, regulatory, and evaluative structures used to govern technologic diffusion in healthcare. Concerns abound that regulatory authorities, IRBs and other ethical bodies, and clinical researchers are all either ill-equipped or under-resourced or being ignored altogether. While many AI-specific oversight frameworks have been proposed to ensure AI diffuses safely, equitably, and reliably, their adoption is piecemeal at best. Meanwhile, market momentum keeps growing. I will review the current landscape of AI diffusion in health and healthcare, review challenges for traditional oversight structures, and discuss how to balance ideal versus practical oversight of what is likely the greatest disruption to health and healthcare in our lifetime.
