Our learning objectives: 

1. Describe the 5Ms framework and apply it to:​

   1. Develop patient-centered care plans for complex patients of all ages ​

   2. Recognize evidence-based practices related to mobility assessment, polypharmacy management, and goals of care conversations​

   3. Foster collaborative decision-making with complex patients and their support aligned with patients’ preferences.

1. Emily Bufkin is Assistant Professor in Internal Medicine and Pediatrics and a proud primary care physician who has the privilege of serving as the program director for UTSW's combined Med-Peds residency program. Her academic work focuses on disability health equity and healthcare transition from pediatric to adult models of care.
