The Dallas Fort Worth Chapter of the American Case Management Association (ACMA) represents case management professionals who live or work in the Dallas area and offers continuing education and professional networking opportunities throughout the area. It is through this organization that three of the area's large hospital systems have worked together to collaborate and break down silos in health care delivery. With the recent increased focus on Social Determinants of Health (SDOH) and Health Equity, ACMA DFW is on a mission to increase awareness about our organization and illustrate how finding your network of people can help you meet the needs of your patient populations. This session will discuss the various ways that each health system represented is working toward the goals of addressing SDOH to achieve Health Equity. We will also discuss the case management standards of care and how you might find case management professionals within the various healthcare systems.
