This clinical feasibility study will investigate the utility of combined PET/MRI using the system L amino acid transport substrate O-(2-[18F]fluoroethyl)-L-tyrosine (FET) for surgical planning and assessing residual tumor in pediatric neuro-oncology patients. Participating patients with brain tumors will undergo an initial FET-PET/MRI study prior to surgical resection and a 2nd FET-PET/MRI after resection and prior to additional therapy. A total of 12 patients will be recruited in this study. Surgical plans based on conventional MRI will be compared to plans using combined FET-PET/MRI, and the percentage of patients who have changes in their surgical plan will be recorded. The percentage of patients with measurable tumor as measured with FET-PET/MRI after surgery will also be recorded.
Patients 18 years of age and younger with known or suspected brain tumors who will be treated with surgical resection will be eligible for participation in this study.

The objective of this longitudinal cohort study is to quantify the effects of antenatal opioid exposure on the trajectory of brain development over the first 2 years of life, examine associations with developmental and neurobehavioral outcomes, and explore how specific factors (differing antenatal and postnatal exposures, severity of neonatal opioid withdrawal, maternal stress/depression/parenting) modify these effects.

3. HCRN Endoscopic Versus Shunt Treatment of Hydrocephalus in Infants (ESTHI): Multi institutional trial. NIH Funded

The ESTHI Trial is a multi-center randomized controlled trial (RCT) comparing endoscopic third ventriculostomy with choroid plexus cauterization (ETV+CPC) and shunt in infants with hydrocephalus. The study will leverage the infrastructure of the Hydrocephalus Clinical Research Network (HCRN), a committed group of 14 leading North American pediatric neurosurgical centers with a long track-record of successful collaborative clinical research and RCTs in hydrocephalus. Optimal cognitive outcome is the primary concern of families and will, therefore, be the primary outcome. Assessment of dMRI, a validated, non-invasive method of measuring white matter microstructural integrity and structural connectivity in the developing brain, will provide further insight into the developmental consequences of these two treatments. The results of the RCT will help families determine the optimal treatment of hydrocephalus for their child.

1. Use of Combination Chemotherapy for Treatment of Granulomatous and Lymphocytic Interstitial Lung Disease (GLILD) in Patients with Common Variable Immunodeficiency (CVID) NIH funded and CHW funded
Principal Investigator: John M. Routes. Radiologist: Sumit Singh, K. S. Shahir, L. R. Goodman
A subset of patients with common variable immunodeficiency (CVID) develops granulomatous and lymphocytic interstitial lung disease (GLILD), a restrictive lung disease associated with early mortality. The optimal therapy for GLILD is unknown. This study was undertaken to see if rituximab and azathioprine (combination chemotherapy) would improve pulmonary function and/or radiographic abnormalities in patients with CVID and GLILD.
