EM2604I "Transition Medicine: The Continuity from Pediatric to Adult-Centered Care" (IM GR-041026)
Purpose and Overview
In this grand rounds, the attendee will learn about the increasing prevalence of young adults with chronic conditions of adult origin who need to transition to adult-centered medical care. A review will be given of the barriers these patients and families and the providers who care for them face. Health policy changes, including payment reforms, to improve transition will be reviewed. Potential solutions will be discussed with a focus on programs in Boston that have been started and preliminary data on transition outcomes.
Target Audience
UT Southwestern faculty, fellows, residents and medical students, community physicians, nurse clinicians, physician assistants and nurses.
Learning Objectives
At the conclusion of this activity, the participant should be able to:
- Understand the growing prevalence of young adults with chronic conditions of adult origin who need to transition to adult-centered medical care.
- Name some of the barriers that patients, families, and medical providers face as they navigate the move from pediatric to adult care.
- Consider potential solutions that can be implemented at UTSW to ease the transition of young adults who are entering the adult health care system.
