Primary care providers recognize the need to control healthcare costs, improve patient outcomes and provide efficient care and as such, seek methods to address these issues in a comprehensive and effective way. The complexity of most traditional health care systems also presents a challenge to deliver care in which the patient’s needs are met in a sustainable way. The patient-centered medical home (PCMH) model holds promise for improving the way clinicians provide primary care. Reported benefits of the PCMH model include reduced costs, decreased staff burnout and improved patient satisfaction. This program will describe the history of the medical home model, the current criteria for an entity desiring to become a PCMH, the published evidence thus far of the benefit of this model and a practical discussion of how one may move their practice towards a patient-centered medical home model.
