       Patient Centered Medical Home for Underserved Patients

This program will describe the history of the medical home model, the current criteria for an entity desiring to become a PCMH, the published evidence thus far of the benefit of this model and a practical discussion of how one may move their practice towards a patient-centered medical home model.

       DEPRESSION 1:  Screening and Monitoring in Primary Care Clinics Treating Underserved Patients

This activity provides awareness of the incidence, predisposing risk factors of depression and its impact on patient outcomes. Primary care providers will attain the foundational competence to identify patients presenting with depression in their settings.

       DEPRESSION 2: Major Depressive Disorder-Remission as a Goal: A Measurement-Based Approach

This training will provide instructions on how to treat depression using a measurement based approach dependent upon how the patient responds to questions on various assessments addressing symptom severity, medication compliance, and overall well-being.

       DEPRESSION 3: Depression Screening Tools and the Basics of Diagnosing

Term: March 1, 2015–March 31, 2017

This training will present standardized screening tools for MDD in detail, including scenarios of provider-patient interaction, as seen in primary care settings. These cases will depict common symptoms of patients with MDD and walk the provider through the proper use of these screening tools to identify and diagnose MDD.

       DEPRESSION 4a: Measurement Based Care for Major Depression: Treatment Decisions and Tactics

Term: March 1, 2015–March 31, 2017

This training provides content to improve the providers’ competence in diagnosing and treating depression, with a goal of remission. Participants will also learn to apply the appropriate screening tool for ongoing assessment of the patient’s status and make corresponding adjustments in treatment.
